Welcome to the online booking system for Dr. Sithembu. Please be advise there are some restrictions to booking you appointment online. Please note if you are booking appointments on behalf of a family member or for a family, please call the clinic to book. One (1) booking is ten (10) minutes long and allows one (1) patient to be seen. All other consults for prescription refills, illness, injections, results, and/or lab work are acceptable to be booked online. Thank you Video Conferencing (Call office for more availability) Starting September 17, 2019 you may book appointment's online for video conferencing. A consent form to communicate electronically is required prior to proceed. We will also need a valid e-mail and your pharmacy details (pharmacy name and fax number specifically) . Dr. Sithembu may see patient's in regards to: - Chronic Prescription for diabetes or hypertension (one refill a year) - Acne - Allergies - Asthma - Bacterial vaginosis - Bites & stings - Body aches - Bronchitis, COPD, ASTHA exacerbations - Cold - Dehydration - Diarrhea - Earache - Erectile dysfunction - Flu - Hives - Lice - Urinary tract infections - Yeast infections What requires attending the clinic to see Dr. Sithembu: Medical emergencies This is not intended for medical emergencies. If you believe you are experiencing an emergency, please call 911 or proceed to your nearest emergency room. Some examples include: - Chest pain - Difficulty breathing - Suicidal thoughts - Major trauma - Gastrointestinal bleeding Controlled Medications Dr. Sithembu cannot prescribe controlled medications such as narcotics online and require a consult in office. Some examples include: - Narcotics, such as oxycodone, sold under brand names such as Percocet and OxyContin - Benzodiazepine medication such as Lorazepam, sold under brand name Ativan and others - Stimulant medications to treat ADHD such as brand anmes Ritalin, Adderall, and Vyvanse Disability & school forms/Sick notes Dr. Sithembu cannot fill out WCB or specialty disability forms, such as school absence forms. These forms tend to be complex, and require an in person consult at the end of the day of office hours. Sick notes will only be provided in an office consult in person during regular office hours. Travel immunization advice Check CDC website for detail travel immunization advice Check-up for WCB/ICBC Patient must come in to the office to follow up their WCB and ICBC cases as these are more detailed visits that cannot be done electronically.
